Job overview

As a band 6 responsible for leadership clinical of patients care delivery. Deputy to the Community ward band 7 ward manager
1. Acting as a patient advocate to ensure safe care delivery as an inpatient leading to developing ensure good discharge planning.
2. Participating in daily board rounds to identify and resolve barriers to discharge, ensuring documentation is accurate and complete.
3. Leading, mentoring, and supporting the ward junior nursing staff
4. Ensuring patient safety during the transition, monitoring infection control, and reporting incidents to maintain high standards of care for all patients.
5. Be able to give good information to patients and carers regarding their inpatient stay, discharge plan, transport and follow-up care.

Main duties of the job

Chartridge ward deputy Sister/Charge Nurse supports and leads in the role the safe, timely, and efficient transfer of patients following the transfer form either BHT acute wards or external trust patients .

The destination supporting could be to home or community care, managing patient flow to optimize bed capacity. Working closely with the wards discharge team Role will include coordinating with the MDT, Documentation, arranging transport and medications. Developing our junior staff and day to day management of a 22 bedded ward

Person specification

EDUCATION, QUALIFICATIONS & TRAINING

Essential criteria

15. 1st Level Registered General Nurse
16. Further professional studies relevant to the clinical area
17. Mentorship programme or equivalent teaching qualification

Desirable criteria

18. Degree in Nursing or Equivalent

KNOWLEDGE and EXPERIENCE

Essential criteria

19. Demonstrate clinical expertise, relevant to the area
20. Demonstrable interest in and commitment to the professional development of others
21. Able to contribute to the development of students, nurses and oneself
22. Able to contribute to develop the workplace as a learning environment
23. Awareness and understanding of current health issues and in particular to demonstrate a good understanding of the NHS Plan and recent initiatives

Desirable criteria

24. Previous experience of leading change in a clinical environment
25. An understanding of HR processes and issues pertaining to staff management
26. An understanding of nursing issues in relation to finance management
