Job Title: Social Worker

Rate: £35 per hour
The Cheshire West and Chester Council is looking for a Social Worker to join their Hospital Discharge team, offering a pay rate of £35 per hour.
Responsibilities:
* Assess the need for social work services, identify risks, and determine intervention methods.
* Advise on the use of social services and resources.
* Liaise with other professionals, statutory, and voluntary agencies to ensure quality service delivery.
* Manage and monitor your workload within policies and statutory obligations.
* Maintain records and carry out administrative procedures.
* Participate in supervision sessions and staff meetings, utilizing training opportunities.
* Contribute to service evaluation and development, sharing knowledge with colleagues and professional groups.
* Supervise students where appropriate.

Qualifications and Experience:
* Social Work England registration
