Responsibilities
* Work as part of a hub and spoke model coordinating a caseload of service users experiencing first episode of psychosis.
* Conduct integrated health and social care assessments and co‑produce care plans with individuals and support networks.
* Implement, review and evaluate evidence‑based, recovery‑oriented interventions in accordance with clinical governance processes.
* Support carers through assessment and provision of appropriate support services.
* Maintain accurate and timely records in electronic systems and produce reports as required.
* Collaborate with multidisciplinary teams, supervising clinicians, and external organisations to deliver high‑quality care.
Qualifications
Academic and professional requirements
* Degree level or above in Social Work or a related field.
* Membership of a recognised professional body and appropriate UK professional registration.
* Working knowledge of MHA (1983), CPA, risk assessment and Section 25, and other relevant legislation.
* Evidence of continuing professional development and short‑course training in mental health.
* Experience in a relevant mental health setting and with electronic recording systems.
* Experience of conducting small‑scale audits and surveys.

* Ability to assess, plan, implement and evaluate agreed care with supervision.
* Excellent communication skills, both verbal and written, across a wide range of stakeholders.
* Ability to manage highly sensitive information securely.
