What you will do:
Conduct assessments of need for children and young people with disabilities, including complex care and safeguarding assessments.
Develop and implement child-centred care and support plans, ensuring services meet the holistic needs of the child and family.
Monitor, review, and adjust care packages, liaising with health, education, and voluntary services to ensure coordinated support.
Safeguard children and young people, identifying risks and initiating appropriate interventions under statutory guidance.
Person Specification:
Qualifications: Degree in Social Work (BA/MA or equivalent) with evidence of continued professional development relevant to children with disabilities.
Registration and Compliance: Registered with Social Work England; knowledge of relevant legislation and frameworks including the Children Act 1989 and 2004, Children and Families Act 2014, and Working Together to Safeguard Children. Enhanced DBS clearance is required.
