Secondary Breast Clinical Nurse Specialist

You will act as a Keyworker for metastatic breast cancer patients, providing a link between patients and healthcare professionals across primary, secondary and tertiary care, and act as an advocate for patients and families.
Kingston and Richmond Hospital NHS Foundation Trust, in partnership with the Royal Marsden Hospital Medical Oncologists, delivers breast cancer treatments.
Core Responsibilities
 * Ensure the role reflects the core components of the clinical nurse specialist: clinical practice, management/organisation, education, research and audit.
 * Continually develop, implement and evaluate the Metastatic Breast Cancer Service to improve and maintain standards of patient care.
 * Act as a clinical expert in breast cancer with excellent communication skills. Work autonomously yet collaboratively within a multi‑professional framework.
 * Participate in the Pan‑London Breast Care Nursing Group.
 * Work a full day on a Wednesday and either a Monday or a Thursday.
Clinical Duties
 * Collaborate with medical oncologists and the acute oncology service (AOS). Refer to specialist palliative care services where appropriate, promoting seamless pathways of care and preventing crisis, inappropriate admissions and reducing inpatient stay.
 * Develop and facilitate Holistic Needs Assessment (HNA) clinics for patients at recurrence or diagnosis of secondary disease, during treatment and at any time individual needs arise.
 * Provide a physical/virtual presence and point of contact for patients with metastatic disease either in person or by telephone contact.
