Job overview

We are looking for a Band 6 Trainee Specialist End of Life Community Nurse to join our Community Nursing teams within the Canterbury Care network (PCN).

The Trainee Specialist End of Life Community Nurse (Band 6) will benefit from a robust 12-month development plan, when achieved you will be awarded the Specialist End of Life Community Nurse Band 7 post. This plan includes the achievement of the Non-Medical prescribing qualification, where the scope must be in end of life care, and also be completed within this 12-month period, if not already held.

As a Trainee Specialist End of Life Community Nurse, you will be responsible for the supervision, teaching and leadership of clinical staff members, and the quality of the care they deliver, as well as holistically managing complex end of life patients during this initial 12-month period, with guidance and supervision in place, and then fully on completion of the 12-month development plan. They will also provide evidence of sound clinical judgement and decision making, and act as a role model for the service and profession

You will provide caseload management within a dedicated team caring for patients within a defined geographical patch in the area, working alongside the Band 6, however, will be required to provide effective clinical leadership across other teams within the Primary Care Networks.

Main duties of the job

The Specialist end of life care nurse will be responsible for the supervision, teaching and leadership of individual staff members and the quality of the care they deliver. The team you work with will be aligned to a Primary Care Network (PCN) where you will represent community nursing and provide secondary line management to support the PCN lead.

The role requires an experienced Community Nurse with non-medical prescribing experience or a willingness to undertake to provide sound clinical judgement and act as a role model for the profession. As the senior clinician you will be a decision-maker and a problem solver with accountability for your team’s performance.

Leadership experience will be invaluable as you support and develop your workforce to meet the varying demands of a busy clinical service, manage staff issues as they arise and help all staff to maintain their own health and well-being.

You will provide clinical expertise and have an overview of the caseloads and their complexity. Experience of managing complex patients where non-medical prescribing is a key element and sharing these skills and knowledge with the team.

Qualifications

Essential criteria

1. Working towards/ Interest in non-medical Prescribing and Advance Practice Skills.
2. Registered health care professional (NMC registration) / significant experience in health improvement environment
3. Health related degree and evidence of relevant post graduate development at Level 6/7
4. Evidence of Post Graduate Specialist qualifications relevant to area of practice (examples: SPDN, Mentorship, Coaching, Clinical Expertise Pathway)

Desirable criteria

5. Management or supervisory qualification/ programme of study.
6. Non-medical Prescribing qualification
7. Advance Practice Skills and Clinical Reasoning qualification

Experience

Essential criteria

8. Experience with the use of Microsoft Office/ eRoster/ Appraisals/ Training Matrix/
9. Significant experience in area of practice
10. Experience of leading and managing teams
11. Experience of supporting the development of protocols and service development plans with practical experience of change management
12. Practical experience in managing health/ team related projects
13. Experience of managing complex caseloads
14. Experience of supporting and being a role model to others (mentor / educator / supervisor) through clinical supervision/ 1:2:1 and personal development plans.

16. Understanding of National Service Frameworks and NICE guidance relevant to care setting and its application across health care services
