Role Description
This is a full-time, on-site role based in Sheffield for a Care Coordinator at Juventa 4 Care. The Care Coordinator will oversee care coordination and case management for clients, ensuring the delivery of personalised and high-quality care. Responsibilities include planning and organising care schedules, collaborating with stakeholders, liaising with clients and caregivers, and addressing any concerns to maintain exceptional service standards. The role also involves fostering strong client relationships and ensuring compliance with care procedures and company policies.
Qualifications
* Proficiency in Care Coordination and Case Management
* Experience in home care and understanding of domiciliary care services
* Strong customer service and communication skills
* Ability to work collaboratively with colleagues, clients, and partner organisations
* Organisational skills and attention to detail are essential
* Relevant certifications or training in healthcare or social care are a plus
