Community Independence Service Urgent Community Response and adjunct pathways for admission avoidance.

CNWL and West London are working together in a Tri-Borough Capacity and this recruitment is a joint campaign to mobilise new service pathways and the development of a hospital at home adjunct to our current UCR services. We work together across the Inner London Borough and our teams work closely together to provide intermediate care services to our populations.

Are you a Band 7 Nurse or Paramedic seeking a new challenging role in the community?

Community Independence Service (incorporating Urgent Community Response (UCR); Hospital at Home and D2A pathways) are currently seeking to employ Band 7 Registered Nurse's or Paramedic's with substantial experience in Acute care (e.g. A&E; Paramedic; ITU; CCU; HDU).

This role is a senior practitioner role who will be clinically competent to lead the team on a daily basis providing visible leadership, as well as working as an autonomous practitioner providing initial assessment and follow care to those in our unoptimized and or optimised patient pathways.

Potential post holders must possess current, valid NMC (UK) or HCPC (UK) registration.

Main duties of the job

Urgent Community Response

Nursing / Paramedic

The main duty of an Urgent Community Response (UCR) nurse/paramedic is to provide, high-quality care to patients in the community who are experiencing an acute deterioration in their health, that could otherwise lead to a hospital admission. UCR nurses/paramedics assess, manage, and treat patients in their own homes or care settings, aiming to review patients within two hours of referral. UCR nurses/paramedics play a crucial role in ensuring patients receive timely medical support, enabling them to recover safely at home and reducing the strain on emergency services

Shifts

The UCR service is a 7 day a week service working hours from 8 am until 10pm and shift patterns are expected and we currently have an 8-8 and 10-10 shift.

Detailed job description and main responsibilities

Some of relevant skills:

1. Physical Assessment (Health Assessment course essential for RGNs)
2. Venepuncture (with ability to identify the required blood tests)
3. Cannulation
4. Medication Administration (PO; PR; IV; IM; SC etc)
5. Triage
6. Autonomous Practice
7. Confidence in knowledge and skills
8. Awareness of the Mental Capacity Act and Mental Capacity Assessment
9. Proficient at taking on a Nurse/Paramedic in Charge role and Clinician of the Day role
10. Non-Medical Prescriber (Desirable)
11. Teaching skills
12. Mentor / Practice Assessor

Within the team, we have Registered Registered Nurses; Paramedics; Occupational Therapists and Physiotherapists and support roles. We work together closely with our OT/PT colleagues, at times undertaking joint patient visits to meet all of their needs.

Successful candidates will have completed a Physical Assessment Module (or a suitable alternative such as the A&E; ITU or DN Course). Paramedics are exempt from this as it's part of the basic curriculum for student paramedics.

Working in this role you will be autonomous but supported by an experienced clinical team, our GP with Special interest and the patients own GP you will be providing care to patients in their own homes who are needing an urgent response to avoid an admission as appropriate, your clinical assessment and decisions will inform the onward care and or escalation of a patient. Confidence in your clinical knowledge is essential. You should be forward thinking and able to prioritise based on clinical need. As a Senior Practitioner, you may be the person others are coming to for advice around decision making too.

Ideally you will possess a Full UK Driving licence as we cover the entirety of the Tri-Borough which includes Westminster/Kensington and Chelsea and Hammersmith and Fulham travel cost within working hours will be reimbursed if a suitable candidate does not have a licence. A car is provided for use on some shifts (automatic gears) but this is for our urgent response so other modes of transport or walking is strongly supported.
