Critical care outreach practitioner

Job summary

The practitioner will assist in the utilisation of an early warning scoring system and respond to requests to provide a specialised assessment of patients who are deemed to be at risk of developing critical illness.

The post holder will be an autonomous practitioner, continually developing a highly specialised knowledge base and clinical assessment skills, working within agreed protocols, as part of a multi‑professional team, supporting staff in the management of patients identified as at risk of clinical deterioration.

They will liaise with medical colleagues to assess, implement, and modify treatment, or to facilitate timely admission to the critical care area.

Patients in critical care will be assessed prior to discharge to general wards, and appropriate guidance and support provided to the multi‑disciplinary team.

The post holder will also play a key role in the delivery and development of formal and informal education to the members of the multi‑disciplinary team.


Main duties of the job

• Undertake specialised clinical assessments of patients at risk of clinical deterioration referred by the multi‑disciplinary team.

• Support staff in managing patients at risk, utilising critical care knowledge and evidence‑based practice, providing specialist advice and support for individuals or the multi‑disciplinary team caring for patients at levels 1‑3.

• Liaise with medical colleagues to assess, implement, and modify treatment, or facilitate timely admission to critical care.

• Assess patients in critical care prior to discharge to wards and give appropriate guidance to the multi‑disciplinary team.

• Maintain high standards of care for acutely ill patients.

• Respond to spontaneous requests for specialist advice, support and guidance.

• Facilitate the timely admission of patients to the critical care areas, coordinating team members to ensure a safe transfer.

• Take responsibility for holding the critical care bleep and respond to emergency calls from wards regarding critically ill patients.


Job responsibilities

The post holder will perform duties as outlined above, including specialised assessments, clinical decision support, and education delivery to the multi‑disciplinary team.

Person Specification

* Registration with the NMC (current PIN)
* Post‑basic qualification to a minimum degree level or equivalent experience
* Post‑basic qualification in Critical Care / Acute Respiratory Care
* Recognised teaching qualification
* ILS/ALS (Resuscitation Council UK) provider
* Sizable Band 6 or senior Band 5 post‑registration experience in a high‑care speciality
* Demonstrate an understanding of the NMC Code of Conduct
* Highly motivated, able to work in a challenging environment, effective change agent
* Professionally motivated and demonstrates professionalism
* Willingness to undertake further relevant study as required for the post
* IT literate
* Collaborative with other disciplines and departments
* Evidence of ongoing personal and professional development
* Knowledge of political and professional issues affecting intensive care service delivery
* Organisational skills
* Independent or team working ability
* Knowledge of audit and research experience
* Understanding of HR policies and procedures, including staff recruitment and selection
* Understanding of the broader local and national healthcare context
* Desirable: Non‑medical prescribing course
* Clinical skills and assessment course
* Current ALS provider, or willingness to undertake the course
