3 months contract with local authority
* Contribute to the effective delivery of an Adult Social Care Service by working as part of an integrated team responsible for safeguarding, support planning, and promoting the wellbeing of adults with care and support needs.
* Identify individual needs through assessment and co-design individual support plans.
* Ensure regular monitoring and reviews of support plans are carried out.
* Manage own caseload and uphold the reputation of the Department and Council while complying with national and service procedures and guidance.
* Undertake work within legislative frameworks, including preparing written reports and representations for Court hearings and applications to the Court of Protection.
* Make complex decisions and manage risks related to caseloads using professional judgment and evidence-based tools.
* Communicate effectively in complex or high-risk situations and represent the department at various forums.
* Work with service users, carers, and families in line with interagency safeguarding adults’ policy and procedures.
* Act as a care navigator for individuals with eligible social care needs and collaborate with health and other agencies to plan relevant responses.
* Ensure the delivery of services within financial resources available to meet assessed needs and outcomes.
* Coordinate, monitor, and review support plans and service standards, identifying gaps in service or unmet needs.
* Work jointly with Social Care Assessors and participate in the Out of Hours AMHP rota if holding an Approve Mental Health Professional (AMHP) qualification.
* Facilitate regular student placements if a practice educator and undertake capacity and best interests’ assessments.
* Lead safeguarding enquiries, work cooperatively with colleagues and in partnership within the Council and across multi-agency boundaries.
* Model inclusive practice in relation to identity and diversity and obtain regular professional supervision to ensure effective practice and career development.
* Prepare and participate in Performance and Development Reviews, identifying areas for improvement and carrying out agreed learning and development opportunities.
* Ensure reports are of high quality and submitted according to appropriate timescales.
* Ensure that client information data is lawfully gathered, accurate, up to date, and only divulged in accordance with relevant regulations.
* Promote non-discriminatory practices in all aspects of work undertaken.
Requirements
* Relevant qualification in social work.
* Registration with Social Work England.
* Approve Mental Health Professional (AMHP) qualification (if applicable).
* Practice educator qualification (if applicable).
* Knowledge of relevant legislation, policies, and procedures.
* Strong communication and decision-making skills.
* Ability to work effectively within a multi-disciplinary team.
