Role Description
This is a full-time on-site role for a Care Coordinator based in Coventry. The Care Coordinator will be responsible for managing and coordinating care plans for clients, liaising with care teams, and ensuring high-quality service delivery. Additional responsibilities include carrying out client assessments, carers assessmenrs, maintaining clear and effective communication with clients and their families, performing case management tasks, and addressing client needs promptly to ensure their overall well-being. Administrative tasks such as record-keeping and updating client information will also be part of this role.
Qualifications
Knowledge and experience in Care Coordination and Case Management
Strong Communication and Customer Service skills
Familiarity with Home Care processes and ability to support diverse individual needs
Excellent organizational and problem-solving skills
Ability to work collaboratively in a team environment
Previous experience in a similar role within healthcare is essential.
Relevant certifications or qualifications in health care, social work, or a related field are an advantage.
