Band 3 Complex Discharge Administrator

The post-holder will provide a comprehensive administrative service for the Complex Discharge Team based within our Complex Discharge Hubs. All activities will underpin the safe and timely discharge of patients and work to reduce patient length of stay. The role involves cross‑site working on a 7‑day rota that includes Bank Holidays.
Main duties of the job
* Receive and process all referrals triggered by ward staff.
* Record and monitor the information in the Discharge Hub Management System (DHMS) and other appropriate systems/excel sheets to keep an auditable referral trail; produce trends, analysis and reports on request.
* Follow up on outstanding information by liaising with wards and Complex Discharge Hub colleagues, recording it in DHMS and templates.
* Provide reports or take minutes as required.
* Provide an administrative and secretarial service to the Complex Discharge Hub Team, including maintaining a diary for the team.
* Order stationery/equipment required by the team using the appropriate system, raise requisitions and receipt invoices where applicable.
* Receive all incoming calls, prioritise and log patient referrals when appropriate.
* Participate in the receipt and distribution of all internal and external mail, logging and/or redirecting as appropriate.
* Liaise with outside agencies and voluntary organisations on behalf of the team (e.g., Care Homes, Social Work Area Offices, GPs, Clinical…).
Job responsibilities
* Commissioning Groups, Mental Health Teams, Courts, Police, Patients and Relatives.
* Liaise with internal staff across ward areas, including all disciplines.
* Participate in annual appraisal with the Senior Discharge Co‑ordinator/Admin Team Leader to identify self‑development and training needs.
* Cover rota basis, particularly during short periods of absenteeism such as annual leave, sickness or training.
Person Specification
Qualifications
* NVQ Level 3 in Business Administration or equivalent qualification and/or experience in an administrative environment.
* RSA Level II or equivalent experience.
Experience
* Experience of working in an administrative environment, ideally in a Healthcare setting.
* Experience of working with the public/customer service.
* Experience of Microsoft packages including Word, Excel, Outlook.
* Experience of minute taking, preparing and distributing agendas for meetings.
* Experience of working in a busy environment.
