Job Overview

THIS POST IS FIXED TERM/SECONDMENT FOR 18 MONTHS DUE TO FUNDING. IF YOU ARE INTERESTED IN APPLYING FOR THE SECONDMENT POSITION, YOU MUST OBTAIN PERMISSION FROM YOUR CURRENT LINE MANAGER PRIOR TO APPLYING FOR THIS POST.


Responsibilities

* Be responsible and accountable for assessment, planning, delivery and evaluation of care programmes for the management of a wide range of patients with lower limb complications in line with national lymphoedema and all Wales Tissue Viability Policy and procedure.
* Manage a clinical caseload of clients with complex nursing needs using evidence‑based, client‑centred principles to assess, plan, implement and evaluate interventions in a variety of settings, through Primary and Secondary Care Services.
* Provide expert specialist support, advice and education to healthcare professionals, patients and relatives/carers in the assessment and management of leg wounds and chronic oedema, ensuring the delivery of evidence‑based best practice.
* Maintain up-to-date expert knowledge and skills in lower limb management.
* Provide leadership, supervision and mentorship for junior staff and deputise for the clinical lead as required.
* Act as an integral part of and an excellent communicator within the multidisciplinary teams for management of Lower limb Wounds/Lymphoedema, taking a key role in the planning and delivery of patient care during an episode of care.
* Make judgements involving highly complex facts or competing demands, e.g. plan and organise provision of care for patients and clients.
* Implement policies and propose changes to working practices or procedures in own workspace.
* Support change and innovation, including service development and developing a case for change.
* Contribute to the development and review of clinical guidelines and policies.


Qualifications and Experience

* Maintain professional registration in line with NMC code and revalidation.
* Hold or work towards a recognised specialist qualification in wound healing and/or chronic oedema management.
* Hold or be working towards a nursing degree.
* Actively participate in continuing professional development to develop new skills and maintain and update existing skills.
* Work within the NMC Code, Health Board, local and national policies and procedures and adhere to patients’ rights.
* Act as a mentor/preceptor to students and junior staff.
* Participate in meeting the learning needs of team members, sharing expertise and evaluating training undertaken.
* Actively participate in Personal Appraisal Development Review (PADR).
* Provide and receive complex, sensitive information and highly confidential information concerning patients or clients requiring empathy, persuasion and reassurance.
* Impart diagnosis to patients and relatives, disclosing highly complex and sensitive clinical information in a format which can be understood and overcoming any barriers to understanding.
* Demonstrate the ability to manage patients with challenging behaviour and apply appropriate management strategies.
* Impart unwelcome news, care of terminally ill/safeguarding issues, dealing with emotionally distressing situations.


Additional Skills

* The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
