Job summary
Broxbourne Alliance has an opportunity for a health and wellbeing coach join our primary care network (PCN), to support our 4 GP practices across the area, on a full-time basis for hours per week, where you will be part of a strong personalised care team.
You will be supporting people with lower levels of patient activation, developing the knowledge, skills, and confidence, by managing their health and wellbeing, increasing patient ability to access and utilise community support whilst providing access to self-management education, peer support and social prescribing.
You will be considering the whole person in addressing existing issues and encouraging proactive prevention of new illnesses. By using this approach you will appreciate patients circumstances in a non-judgemental manner, resulting in the need for positive communication and negotiation skills that will be supporting the patients personal choices.
Main duties of the job
1. working in a patient facing role to assess & treat patients using expert knowledge.
2. be responsible for the assessing new patients with subsequent production & completion of individual care plans.
3. coaching and motivating patients through sessions, identifying needs, setting goals & supporting them to implement their personalised health & care plan.
4. providing personalised support to individuals, families & carers, ensuring that they are active participants in their own healthcare.
5. being responsible for referring patients who require the intervention of other healthcare professionals.
6. working with patients and understanding their level of knowledge, skills & confidence when engaging with their health & well-being.
7. providing support to local community groups & work with other health, social care & voluntary sector providers to support patients needs.
8. identifying risk factors & categorising patients into appropriate risk banding.
9. being responsible for the day-to-day planning of personal workloads
10. liaising with the practices & standardising processes across the PCN.
11. receiving referrals & directing patients from triage services & other clinicians.
12. actively signposting patients to the correct healthcare professional.
13. maintaining accurate clinical records of all patient consultations.
14. promoting health coaching and social prescribing.
15. participating in the management of patient complaints.
16. contributing to public health campaigns ( flu clinics) through advice or direct care.

Broxbourne Alliance is a busy Primary Care Network supporting four local practices; Abbey Road Surgery, Cuffley and Goffs Oak Medical Centre, The Maples Health Centre, Warden Lodge Medical Centre. We are a forward thinking PCN covering a diverse patient population of around 45,000 patients.

25. NVQ in Health and Social Care (preferred)
26. Be an experienced health and wellbeing professional, confident to deliver support and advice to a range of patients with diverse health needs.
27. Have strong experience in supporting patients with long-term conditions and a lack of knowledge, or skills, to improve and manage their own health and wellbeing effectively.
28. Have excellent interpersonal and communication skills.
29. Be confident to be involved in multidisciplinary meetings.
