Advanced Nurse Practitioner/Advanced Clinical Practitioner/Trainee, Band 7Annex 21 Para 2 (i)

With effect from 01 April 2026, NHS Scotland is reducing its full time working week from 37 to 36 hours per week, there will be no change in full time pay.


An exciting opportunity has arisen to join our Advanced Practitioner team within Perth & Kinross Health & Social Care Partnership This post will work within the Urgent Care GP Home Visiting Service as part of our Primary Care Team.

The Advanced Nurse Practitioner role within offers individuals the opportunity to develop their career in advanced practice; improving patient outcomes and pathways of care for older adults in primary care.

The post holder will have experience of working at Advanced level within a clinical setting, demonstrating excellent leadership and communication with the ability to work autonomously across the four pillars of advanced practice. Applicants will have the ability to provide clinical leadership through comprehensive patient history taking, advanced clinical assessment, differential diagnosis, problem solving and treatment planning incorporating non-medical prescribing.

The post holder will play a key role in the clinical leadership and development of advanced practice within primary care, delivering high quality evidence based and person-centred care to elderly patients within the community.

You will pro-actively manage older adults and you will be expected to act as a clinical leader working across general practice in close collaboration with the General Practitioners and the Integrated Care Teams.

This role will be fully supported by the Lead Advanced Nurse Practitioner and candidates will be supported to develop by working collaboratively with all members of the community multidisciplinary teams.

Evidence of continual professional development within an appropriate clinical setting, the proven ability to work toward Masters Level learning in Advanced Practice and a qualification as an Independent Non-medical Prescriber is essential. The applicant will also need to demonstrate extensive and varied post registration experience and be able to travel across Perth and Kinross to undertake the role. Candidates who do not meet the Advanced Nurse Practitioner criteria are welcome to apply for the trainee role and will be fully supported to undertake the required education and supervised practice to develop the competence and confidence required to work as an Advanced Practitioner. It is an expectation that as a trainee you will successfully achieve the required qualifications as an Advanced Nurse Practitioner within the first 18 to 24 months of appointment.

Successful candidates will play a key role in enhancing person centred care and experiences, assuring clinical governance and supporting the Lead Advanced Nurse Practitioner, ultimately improving outcomes for patients in the community.

You must be registered on Sub-part 1: RN1 or RNA or Sub-part 2: RN2 of the UK NMC Register or the Health and Care Professions Council (HCPC).
