As a member of the clinical team the ANP will undertake consultations with patients either face to face or via the telephone, making professional autonomous decisions within their scope of practice.
The individual will be able to prioritise health problems, with the ability to intervene appropriately to lead or assist in complex urgent or emergency situations.
The ANP will assess patients by taking a clinical history and examination if clinically indicated.
A clinical management plan will be formulated utilising evidence based practice and patient preference.
As an independent prescriber, medications will be prescribed and reviewed to ensure concordance, monitoring and titration of treatments in accordance with local and national guidance.
Whilst undertaking a holistic assessment the ANP will opportunistically undertake health promotion and preventative care as clinically indicated.
Patients will be referred to other health care practitioners as deemed necessary.
These may be internal referrals, to the community team or to secondary care services.
Accurate contemporaneous records will be maintained electronically.
