We are currently recruiting for full‑time Lead Reablement workers in Ely, Cambridgeshire. The Reablement Service provides care at home to people aged 18 and over, aiming to maximise their independence through short‑term, person‑centred support.
Key Role
Lead Reablement workers are the first point of contact for service users, working closely with Occupational Therapy teams and other health professionals to set personalised goals. They guide users through the reablement journey until independence is achieved, refer to community services, and monitor care continuously.
Responsibilities
* Assess individual needs and create tailored reablement plans that focus on recovery and independence.
* Work hands‑on with service users, helping them relearn everyday living skills such as personal care, mobility, and meal preparation.
* Monitor progress and review plans, ensuring goals are met and adapt support as needed.
* Collaborate with health and social care professionals, families, and other agencies to provide a holistic approach to care.
* Maintain accurate records and documentation to ensure compliance and quality standards.
* Manage a varied caseload to deliver timely, high‑quality support and positive outcomes.
Qualifications
* Proficient written and spoken English for case recording.
* NVQ Level 3 or equivalent apprenticeship.
* Experience supporting people with care and support needs.
* Knowledge of The Care Act and CQC requirements.
* Strong communication skills, able to oversee Support workers and liaise with multiple professions.
* Ability to manage and prioritise a diverse caseload in a fast‑paced environment.
* Excellent verbal, written and IT skills.
* Aware of equality, diversity and inclusion, and able to maintain safety and confidentiality.
* Access to a car, a full driving licence and business insurance.
