Health Improvement Coordinator

The Weight Management Single Point of Access (SPOA) will be provided as a pilot service for 18 months, serving as the gateway to appropriate interventions for weight management across Devon. The team will receive all referrals related to weight management in the region.

Main duties of the job
* Act on the outcome of clinical triage of referrals and manage onward referral processes for non‑complex service users, following clinical referral guidelines and pathways.
* Contact referrals to discuss options, ensuring all service users receive up‑to‑date information on a range of health improvement programmes, with a primary focus on weight management initiatives.
* Provide support to service users through health coaching conversations to help them navigate their referral options.
* Support the Single Point of Access team and patients who are referred into the service.
* Assist with screening of patient questionnaires to aid appropriate signposting for onward care.
* Manage referrals through the electronic referral system (ERS), ensuring they are actioned within expected timeframes.
* Follow up with service users who do not respond to communications/referrals.
* Perform administration duties as required.
* Support audit and evaluation of the Single Point of Access pilot programme.
About us
Based at St Edmunds Community Care Centre in Torquay, with the potential to travel across Devon, the Devon Weight Management Single Point of Access Team is a new service comprising a Service Lead, a Health Improvement Practitioner (this post) and a Health Improvement Coordinator.
The aims of the SPOA are:
* Improve access to weight‑management support and, where clinically appropriate, specialist weight‑management services.
* Standardise triage and referral processes.
* Reduce duplication across weight‑management and associated pathways.
* Link pathways for patients with long‑term conditions where weight management is recommended (e.g., diabetes).
* Evolve to become a single access point for weight management across all levels of support (tiers 1‑4).
Job responsibilities
* Support clinical triage for referrals into the weight management SPOA service.
* Provide advice and guidance to people regarding their weight‑management options, including complex presentations (in consultation with the service lead).
* Support onward referral for service users to local or national weight‑management programmes.
* Manage caseload for self and allocate to Health Improvement Coordinators.
Person Specification
Qualifications
* Evidence of continuing professional or personal development in health improvements, behaviour change or public health to a degree level or equivalent.
* Evidence of competency in IT skills.
* Health promotion certificates such as Brief Interventions for Alcohol, Mental Health First Aid.
* Health Coaching qualification.
Knowledge
* Knowledge of the benefits of health improvement interventions.
* Knowledge of monitoring and evaluation methodology.
* Demonstrable experience of working with a wide range of people and groups using motivational interviewing and behavioural change.
* Experience of using office‑based software (e.g. word processors, spreadsheets, databases) in a workplace setting.
Skills
* Ability to work both autonomously and as part of a team, managing own caseload.
* Experience of using clinical systems.
* Understanding of audit and evaluation.
