This is a newlycreated and novel post at HHP. Working alongside our Triaging GP team andUrgent Care Team, this role will support "same day" and"soon" patient access through a variety of appointment modalities,for example, telephone/Face to face/video and home visits (where needed). Thisrole supports the parameters of the recently announced 2026/27 GP Contract.
We are a TotalTriage Practice meaning all appointments will be RAG rated and work-flowedprior to booking. As a GP within our Urgent Care setting, this role will suitthose who are newly qualified and/or those with experience. Given the evolvingnature of GP urgent access, cases may range from simple acute illness tocomplex co-morbidities requiring the full range of GP expertise.
This role isnon-list carrying, meaning that there is less "behind the scenes"administrative work to address so the role if more focused on patient-facingactivity.
Furthermore, the post holder will adhere to the GMCstandards for good medical practice, contributing to the effective managementof the practice, leading by example, maintaining a positive, collaborativeworking relationship with the multidisciplinary team.
Main duties of the job
The following are the core responsibilities of theSalaried GP. There may be, on occasion, a requirement to carry out other tasks;this will be dependent upon factors such as workload and staffing levels:
The delivery of highly effective medical care to theentitled population
The provision of services commensurate with theprimary care contract
Generic prescribing adhering to local and nationalguidance
Effective management of long-term conditions
Deal with patients who need on the day appointments orwho need to be seen within 48 hours
Maintain accurate clinical records in conjunction withgood practice, policy and guidance
Work collaboratively, accepting an equal share of thepractice workload
Adhere to best practice recommended through clinicalguidelines and the audit process
Contribute to the successful implementation ofcontinuous improvement and quality initiatives within the practice
Accept delegated responsibility for a specific area(or areas) of QOF
Attend and contribute effectively to practice meetingsas required
Contribute effective to the development andmaintenance of the practice including clinical governance and training
Ensure compliance with the appraisal process
Support the training of medical students
Special interests/ skills are encouraged
Undertake Private work where applicable e.g. To whomit may concern letters and charge according to the Practice fees list
About us
Hart Health Partnership is a large Practice in Fleet, Hampshire, serving a list of 28000 patients. We have a diverse multi-disciplinary team with 22 GPs, 11 Nurses, 4 paramedics, 1 ANP as well as other allied colleagues such as First Contact Physiotherapists, Pharmacy colleagues, Mental Health Support Worker and a large care navigation and admin team.
We arelooking for 4 sessions per week, working Mondays and Tuesdays.
