Specialist Support Worker
Location: Abbey Foregate, Shrewsbury
Salary: £24,706.50 per annum
Hours: 37.5 hours per week
About the role
We are seeking a Specialist Mental Health Support Worker at our supported living rehabilitation service in Shropshire. The role involves providing rehabilitation-focused support to individuals with complex and enduring mental health needs, working within a 24-hour service. You will support customers with life skills, promote independence, and help them build successful relationships within their community, enhancing their health, well-being, and daily living skills.
Key Responsibilities
* Deliver high standards of support, demonstrating good practice.
* Promote independence and choice for each customer.
* Assist with personal hygiene and appearance as needed.
* Support social, leisure, work, and educational activities, including using ICT to access information and services.
* Help customers with meal preparation, cooking, menu planning, and budgeting.
* Encourage active community participation.
* Support customers in moving on from services, liaising with care teams.
* Establish links with healthcare professionals and agencies.
* Undertake key-working, including support plan reviews and meetings.
* Conduct assessments in collaboration with customers and agencies.

About You
We seek a motivated individual eager to deliver quality support to vulnerable adults. You should:
* Be flexible to work within a 24-hour rota, including evenings, weekends, and sleep-in shifts.
* Have experience supporting individuals with mental health issues such as self-harm, personality disorders, and schizophrenia.
* Be self-motivated and able to work independently.
* Have experience in partnership working and creating personalized support plans.
* Be skilled in risk assessment and management.
* Be proficient with Microsoft Office.
* Hold NVQ Level 2 in Health and Social Care.
* Have supported housing experience.
