We are looking to recruit a care professional with a keen eye for detail. The ideal candidate will be experienced in writing care plans, and managing their understanding and execution in the community as well as being able to lead by example.
Pay Rates:
1. £23,000pa
2. Out of hours shift payments - £20 M-F & £160 weekends
3. 45p tax free mileage rate
Shifts:
4. Full time Monday to Friday – 9am – 5pm
5. There will be paid on call duties on a rolling rota basis.

Requirements:
17. Clean and valid driver's license with access to a reliable vehicle.
18. A Level 3 NVQ qualification in adult and social care or equivalent is desirable.
19. Proficiency in written and spoken English with strong communication skills.
20. Experience in care plan management.
Key Responsibilities:
21. Work closely with our Reablement Team Manager, Coordinator, and other professionals involved in the care and support of our service users to develop care plans.
22. Co-design care and support plans with customers and their families, ensuring their personal care needs are met, dignity is maintained, and individual outcomes and preferences are reflected.
23. Conduct relevant risk assessments to ensure the highest quality of care.
24. Promoting and facilitating client independence.
