Job responsibilities
* Physical health reviews: ECG, phlebotomy, BMI, physical observations and lifestyle assessment.
* Attend MDT and liaise with agencies to improve health.

Person specification
* Registered Nurse Associate.
* Full driving licence and access to a car.
* Physical Health experience.
* Phlebotomy trained.
* ECG trained – completion.
* Community experience.
