Position: Social Prescribing Link Worker
Location: Birmingham based in GP Surgeries
Hours: Full time 37hrs and part time post 22.5hrs p/w over 3 days ) flexible working available
Contract: 2 post available - 1 x FTE (job share may be considered) & 1 x Part Time
Salary: £27,619 per annum pro rata for part-time

The Role
Social prescribing link workers will work as a key part of the primary care network (PCN) multi-disciplinary team.
