The post holder will be based at C&E Centre, Chelmsford, and will hold a caseload of service users with moderate to severe mental health problems, working within the agreed model of care for the service. The post holder will work as an accountable practitioner under the supervision of senior staff. This role involves assessment, planning, implementation, and evaluation of the care provided, along with effective liaison with all agencies involved with the client or service user.

The post holder will be supported by a multidisciplinary team comprising nurses, doctors, psychologists, peers, occupational therapists, employment specialists, etc.

Care coordination for own allocated patients will involve assessment, planning, implementation, and evaluation of the care provided, with effective liaison with all involved agencies.

The role requires effective liaison with other agencies for case discussions and joint working to develop treatment plans for the client group.
