Job Summary:
* The Access Screening Officer provides frontline contact for adult social care enquiries, handling referrals and delivering high-quality advice, screening, and signposting.
* This role supports individuals with a range of health and social care needs by offering accessible, empathetic, and accurate information via multiple communication channels.
* It involves assessing eligibility for services, identifying risks, and allocating or recommending appropriate resources, ensuring safeguarding and promoting independence.
Key Duties/Accountabilities (Sample):
* Act as the first point of contact for adult social care enquiries via telephone and other digital channels.
* Carry out initial screening and information gathering for new referrals based on predefined criteria.
* Provide clear and accurate advice, guidance, and signposting to customers, carers, and professionals.
* Assess customer eligibility for services, concessions, and support funding using judgement and policy guidance.
* Identify safeguarding risks and escalate or allocate resources accordingly.
* Maintain up-to-date records in line with Data Protection and Information Governance standards.
* Contribute to the improvement of customer experience and service delivery through feedback and system use.
* Support the training and induction of new or less experienced staff as needed.
* Attend team meetings and participate in the development of procedures and best practice.
* Promote the use of preventative services and support customers to access self-help and community-based solutions.
Skills/Experience:
* Strong verbal and written communication skills, with the ability to manage sensitive or complex conversations.
* Proven ability to prioritise and manage workloads independently in a fast-paced environment.
* Competency in using IT systems, including databases, email, and customer management platforms.
* Experience working in a health or adult social care setting with vulnerable adults.
* Understanding of adult social care policies, safeguarding principles, and eligibility criteria.
* Ability to resolve issues effectively and handle escalated enquiries with professionalism and empathy.
* Commitment to equality, diversity, and always maintaining confidentiality.
* Knowledge of Telecare, basic equipment, or assistive technologies that support independent living.
* Trusted Assessor qualification or ability to assess for simple enabling equipment.
Additional Information:
* The closing date: 30/09/2025 @13:00.
* Working Hours: 35 hours per week, Monday–Friday, typically 09:00–17:00.
