This is a full time, permanent role and offers a salary of £23,400.

As a Case Work Coordinator, you will play a pivotal role in managing referrals, assessing client needs, and providing comprehensive support to maintain accommodation and facilitate positive life changes.
Conduct needs and risk assessments, developing tailored support plans.
Maintain accurate client records and contribute to data analysis for service improvement.
Proactively manage neighbourhood and community relations.
Participate in the on-call rota system.
Understanding of homelessness causes and social exclusion, excellent communication skills, and proficiency in Microsoft Office.
Have a full driving license and access to a car for work purposes
