This role involves the day-to-day delivery of care within our Integrated Community Stroke Multidisciplinary Team, comprising AHPs, dedicated to making a difference in the community we serve.
Main duties include:
1. Demonstrating clinical and professional expertise to ensure individuals with speech, language, communication needs, and/or dysphagia benefit from a coordinated, specialist, and effective speech and language therapy service.
2. Supporting clients to live independently and manage their health needs post-stroke.
3. Working collaboratively within a multidisciplinary team to provide compassionate, timely support to diverse clients and their families.
4. Exhibiting excellent communication and interpersonal skills, being adaptable, reliable, resourceful, and approachable, establishing effective relationships across various settings.
