An exciting opportunity has arisen within the Kingsley Ward Team, our inpatient dementia assessment unit at Hollins Park Hospital, Warrington. Our ward provides high standards of assessment and treatment for those with Dementia and BPSD (Behavioural and Psychological Symptoms of Dementia).
You will gain valuable experience and skills across the field of Dementia, incorporating both physical and mental health. You will be motivated to make a real difference in the lives of people living with dementia, where passion, empathy, strong communication, and excellent teamwork are essential qualities for this role.
Key Responsibilities:
1. Coordinate the care of service users, carrying out assessment planning, implementation, and evaluation of their care under the guidelines of the care programme approach and under the supervision of the line manager.
2. Ensure that the service user's care plan is developed to meet their assessed needs and that unqualified staff deliver care as laid down in the care plan.
3. Liaise with family members, carers, and significant others when completing assessments and developing care plans.
4. Develop a risk management plan for service users, considering contributions from other team members, and ensure all significant people are aware of and action the plan.
5. Direct unqualified staff to ensure the delivery of high-standard care to service users.
