An exciting opportunity has arisen for an experienced and enthusiastic nurse to join our Children in Care Team as a Band 6 – Children in Care Nurse (30 hours). The ELHT Children in Care Team is part of the dynamic Community Neuro disability Paediatric (CNP) Directorate, Family Care Division. We work with Community Neuro disability Paediatricians providing a specialist service to complete statutory Initial Health Assessments with Children in Care (0-18) across East Lancashire and Blackburn with Darwen.

We require a caring, highly motivated, enthusiastic nurse with a specialist interest of Children in Care. The post holder will be based at Great Harwood Health Centre.
Main duties of the job
Excellent communication skills to liaise with the child/young person’s professional care network including child/young person’s Social Worker, Health Professionals, Foster Carers to gather the required health information, and support their attendance to ensure the health assessment is of good quality and is completed within statutory timescales.
The successful candidate will work directly with Children in Care, their families/carers and alongside the Community Neuro disability Paediatrician to assist with the holistic completion of Initial Health Assessments (IHA) for Children in Care to ensure compliance with statutory requirements. This will include the effective implementation of statutory health assessments, identification of any unmet health needs and develop individual health care plans.
The successful candidate will provide expert clinical advice, deliver health promotion advice, deliver health education and training as required and work within a public health model which incorporates identification of the health needs of Children in Care.

Detailed job description and main responsibilities
The post holder will work within the Community Neuro disability Paediatric (CNP) Directorate as part of the Family Care Division. CNP is commissioned to complete statutory health assessments for children who are in care. You will work alongside the Community Neuro disability Paediatricians across East Lancashire and Blackburn with Darwen to identify unmet health needs and develop a plan of intervention or ongoing referral to ensure that the health needs of children who are in the care of the Local Authority care are assessed and outlined.
Responsible to the Children in Care Lead Nurse you will work closely with Community Paediatricians, Parents/Carers, universal School Nursing and Health Visiting teams, Local Authorities and Children in Care teams.
Will assist with the holistic completion of Initial Health Assessments (IHA) for Children in Care to comply with statutory requirements.
To review, summarise and interpret health records and input into the IHA to promote the development of a meaningful, holistic health care plan.
MAIN DUTIES
- Review, summarise and interpret health records to input into IHA for Children in Care.
- Analyse holistic health chronologies and contribute to written comprehensive reports detailing the implications of the information for the child’s current and future health and wellbeing.
- Provide support and reassurance to families and carers acting as a key conduit and contact point between family/carers and health professionals.
- Provide support and reassurance for the child/young person answering questions and allaying fears.
- Support the formulation of meaningful individual healthcare plans/adoption reports and ensure they meet the standard required for Quality Assurance.
- Have an understanding of Quality Assurance measures and processes.
- Maintain up to date knowledge of legal processes and requirements for children in care.
- Ensure the service meets national targets for Initial Health Assessments identifying any gaps.
COMMUNICATION
- Demonstrate the ability to listen and engage in therapeutic practice.
- Able to communicate effectively with children, young people, families and carers including those with additional needs utilising resources where necessary.
- Ability to adapt and be sensitive to the needs of the child, young person, family and carer.

TRAINING AND DEVELOPMENT
- Work with other Paediatric Service members to develop the service and continuous evaluation to ensure that the most up to date and appropriate processes are used.
- Engage in effective strategic planning of services for Children in Care.
ORGANISATIONAL RESPONSIBILITIES
- Ensure that appropriate resources are available at all times.
- Ensure the safe use and maintenance of equipment.
- Ensure that patient outcomes are recorded in a timely way as per Trust policies and processes.
- Provide quality written reports for patients, parents/carers and other professional within the Trust processes.
- Develop resources that could/can be used in the deliverance of services.
- Keep up to date with relevant legislation and statutory requirements for Children in Care.
PROFESSIONAL RESPONSIBILITIES
- Actively liaise with other professionals within the Caldicott data protection and information governance.
- Maintain practice within Nursing, Midwifery Council Professional Code of Conduct
- Ensure accurate record keeping as per Trust policy and NMC Guidelines on paper and electronic patient records
- Keep up to date in line with Intercollegiate Document/Working Together and other relevant guidance

Essential criteria
* Qualifications First Level RN (RSCN/SCPHN-School Nurse/SCPHN-Health Visitor)
* Significant experience (ideally 3 years) of working with Children in Care Significant experience of working with children and young people Recent experience of working with children with additional needs/complex medical needs Experience of working in community setting Evidence of leadership qualities
* Knowledge and Skills Knowledge and experience of Children in Care, including Statutory legislation/guidance Up to date knowledge and experience of Safeguarding legislation for children and young people Experience of completion of Review health Assessments for Children in Care
