Job overview

Are you a Staff Nurse within an acute care setting looking for an opportunity to work within a Community Care setting whilst utilising your acute care skills to deliver a unique service that aims to bring hospital care to the Patient within their own home? If so, this unique role is for you.

The Acute Care at Home service is a partnership between the University Hospital of North Midlands (UHNM), University Hospital of Derby and Burton (UHDB), Royal Wolverhampton Trust (RWT) and Midlands Partnership Foundation Trust (MPFT). The successful applicants will join an integrated, collaboratively focused team, employing proactive clinical practice and knowledge, toprovide clinical assessment, monitoring and support, as part of our urgent carepathways.

As a Staff Nurse working within the North Staffordshire team, you will play a key role in supporting hospital admission avoidance and supporting patients to leave hospital sooner than would otherwise be possible.You will deliver high quality, professional evidence-based care when visiting patients across North Staffordshire.


Main duties of the job

The post holder will work autonomously under the guidance of the Senior Sister/Charge Nurse and be responsible for the clinical assessment of a designated group of patients on the Virtual Ward.

They will carry out face to face visits to deliver care and assess response to treatment plans to inform consultant led programmes of care, always maintaining quality and safety standards.

As part of the role, they will utilise skills such as delivery of IV medications and health monitoring for patients with a wide range of medical conditions across North Staffordshire. They will act as a professional role model to staff and service users contributing to staff education and development plus quality improvement initiates and service development.

You will be required to have access to a vehicle which can be used for work purposes or able to travel as required to meet the needs of the service. Please note not all office locations or client care bases may be reached by public transport?

Detailed job description and main responsibilities

* Perform a wide range of tasks such as administration of IV medications, venous cannulation and venepuncture, urinary catheterisation, perform ECGs.
* Direct clinical assessment and care provision of people in their home environment, who have been identified as meeting the Virtual Ward admission criteria.
* Participation in daily Multi-Disciplinary Team meetings and Consultant ward rounds, ensuring that patient related information is up to date and information is disseminated accordingly.
* Act as a point of contact providing advice, support and guidance to patients, relatives, colleagues and other professionals on issues relating to care.
* Communicating sensitive information concerning patients’ medical condition(s) and status which involves relationship building, displaying persuasive and reassurance skills always acting in the best interests of patients.
* Recording and communicating information relating to patient care in a clear and concise manner to a high standard.
* Demonstrating skills to assess and interpret specialist acute and other patient conditions within the patient’s home environment, initiating actions as appropriate.
* Management of medications, including providing advice and guidance, administration of Intravenous medications.
* Perform diagnostic testing including venepuncture, ECGs, Urinalysis, peak flow.
* Dealing with distressed relatives, care of the terminally ill and with the consequences of terminal illness.
* Contributing to the formal induction of all new staff and the development of staff training programmes.
* Contribute to the principles of clinical governance ensuring that quality standards are set and monitored.


Person specification


Qualifications

* Current NMC registration
* Degree level study in relevant field


Experience

* 1 years post registration experience
* Experienced in venepuncture
* Experience in an acute care setting
* Experienced in Venous Cannulation
* Experienced in IV medications administration
* Experience in an Acute Care setting
