Overview
The post holder will support individuals leaving prison to enhance their health, wellbeing and quality of life, assisting them to connect with and access health services, voluntary sector services and community-based activities and support.
Responsibilities
* Support individuals leaving prison to engage with health services, voluntary sector services and community-based activities and support to improve health and wellbeing.
* Receive referrals from Criminal Justice Agencies, individuals and families.
* Early identify the needs of people who are vulnerable and who may struggle to engage with community-based health services due to comorbid health problems or other significant health or social care needs.
* Coordinate and interconnect with health and community service providers; monitor and record service activity and outcomes for individuals and families.
Referral & Collaboration
The focus of the role includes interlinking with health and community service providers as a fundamental part of the work, ensuring appropriate connections and follow-up.
