Overview
St Martins has a vacancy for an Integrated Housing and Care Coordinator. This is a fixed term, full-time contract until 29th March 2026, 38 hours per week.
The main purpose of the job is to deliver the care and support needs of people we house through the Ketts Hill project, for people with multiple and complex needs who face barriers to existing general needs accommodation and have been sleeping rough.
Responsibilities
* Support people facing rough sleeping and multiple and complex needs to access suitable housing and care services.
* Demonstrate emotional resilience and tenacity to provide support, even when faced with resistance.
* Engage with individuals who have poor mental health, addictions, or who are ex-offenders, delivering person-centred support.
* Be flexible in approach to meet changing needs and circumstances.
* Show confidence, adaptability, conscientiousness, empathy, and determination to make a difference.
* Integrated Housing and Care Coordinator (role title in simple text form).
