We are seeking an enthusiastic and committed staff member to join our community service. The service aims to provide recovery-focused treatment and support through personalized care plans for individuals with mental health disorders, supporting them from diagnosis to recovery to enable them to lead fulfilling lives. You will work with a Multi-Disciplinary Team including Nurses, Psychologists, Doctors, Social Workers, Occupational Therapists, and support staff. A key aspect of the role involves undertaking physical health assessments.
Main duties of the job
* Collaborate with colleagues, service users, and carers to ensure focused care and treatment.
* Work with individuals experiencing mental health conditions who may be vulnerable.
* Support physical health assessments; training will be provided.
* Work as part of an established multidisciplinary team under the guidance of qualified practitioners to deliver individualized support per care plans.

Job description and responsibilities
* Collaborate with partner organizations, service users, carers, clinicians, and other practitioners to deliver high-quality, recovery-focused care.
* Assist in managing and caring for service users based on individual needs, under supervision.
* Support physical health assessments and provide practical support, guidance, and education to service users.
* Maintain accurate documentation and records in line with Trust policies.
* Conduct risk assessments and report concerns to senior staff.
* Attend weekly multidisciplinary team meetings.
The Mid and South Essex ICS aims to improve health and wellbeing through integrated services, including the HCA Academy, which supports onboarding, induction, and training for Healthcare Assistants. The successful candidate will attend a 5-day face-to-face induction at Brentwood Community Hospital, with start dates discussed during recruitment.
