Band 6 - Specialist Respiratory Practitioner
We are seeking a dedicated and experienced Specialist Respiratory Practitioner to become a vital part of our dynamic and supportive team, working 22.5 hours per week.
Responsibilities
* Collaborate with a multidisciplinary clinical team to deliver exceptional care and support.
* Conduct thorough respiratory assessments and create personalised care plans.
* Monitor patient progress and adjust interventions based on clinical evidence and feedback.
* Educate and support patients and families to promote self‑management of respiratory conditions.
* Coordinate with colleagues for person‑centred, coordinated care.
* Contribute to service development initiatives and clinical audits.
* Assume a case‑management approach to prioritise ongoing patient care.
* Maintain accurate records using IT systems and support clinical audits.
* Ensure compliance with professional and organisational policies and procedures.
Main Responsibilities
* Develop and implement care plans based on comprehensive patient assessments, current guidelines, and best evidence.
* Assess patient suitability for Long‑Term Oxygen Therapy (LTOT) and ensure cost‑effective and appropriate provision.
* Provide specialist assessment and treatment for patients with COPD and ILD.
* Support the delivery of Community Pulmonary Rehabilitation (PR), including education and exercise programmes.
* Promote patient self‑management through behavioural change strategies.
* Communicate complex clinical information to patients, families, and healthcare professionals.
* Use IT systems to maintain accurate records and support clinical audits.
* Work collaboratively within the community nursing team and multidisciplinary services.
Qualifications
* NMC Registration.
* Proven experience in respiratory care or a related specialty.
* Strong assessment and clinical decision‑making skills.
* Commitment to continuous professional development.
* Excellent communication and teamwork abilities.
