Belmont Recruitment are currently looking for an experienced Leaving Care Worker to join Lincolnshire County Council on an initial 3 month temporary contract. This is a full-time role working 37 hours per week, Monday to Friday. About the Role: As a Leaving Care Worker, you will be a consistent and reliable point of contact for young people leaving care, coordinating support to ensure a smooth and safe transition to adulthood. You will work directly with young people, their families, and a range of partner agencies to build and deliver effective support plans tailored to individual needs. You will: * Manage a caseload of care leavers, working in partnership with other agencies. * Support young people to access education, employment, training, and stable accommodation. * Help develop independent living skills, improve wellbeing, and reduce risk factors such as antisocial behaviour or poor mental health. * Provide a flexible response, including evening and weekend work as required. * Act as a lead professional in delivering Pathway Plans, ensuring measurable progress and positive outcomes. What We’re Looking For: * Have an NVQ Level 3 qualification (or equivalent) in a relevant area. * Understand the challenges and legislation relating to care leavers. * Can build strong, respectful relationships with young people and professionals. * Are confident in coordinating multi-agency support and delivering interventions. * Have excellent record-keeping and case management skills. * Hold a full UK driving licence and have access to a vehicle for work purposes.
