The Practice Nurse will act autonomously within their professional scope of practice providing care for patients face to face, telephone consultations and home visits. You will be responsible for the provision of medical services to our patient population managing a caseload dealing with a wide range of health needs. You will be supporting & supported by the wide range of clinical and non-clinical team within the practice. Main duties of the job Manage a defined caseload of patients Deliver chronic disease management (e.g. diabetes, COPD, asthma, hypertension) Provide immunisations, cervical screening, wound care, and health promotion Contribute to QOF achievement and clinical audits Work collaboratively with the wider clinical and administrative team About us We are a very warm and welcoming practice of over 6,000 patients, providing quality healthcare for our patients in Oldham.
