An opportunity has arisen for a clinical pharmacist to join South Uttlesford PCN, to provide high quality patient focussed pharmacy services to the patients of the PCN. This position is offered on a full-time basis and based in/out of a variety of South Uttlesford practices/locations according to need. Main duties will include; Patient facing long-term condition clinics to review the ongoing need for each medication Patient facing clinical medication reviews Patient facing care home medication reviews Patient facing and telephone medicines support Unplanned hospital admissions – review of medications and reconciling of medications Repeat prescribing Identification of high risk patients Please see job description for further details. About the Candidate You will be a qualified pharmacist with strong experience within primary care and have the following skills and qualifications: As a minimum, you will hold a degree in Pharmacy (Masters or equivalent) Be registered with the General Pharmaceutical Council and a member of the Royal Pharmaceutical Society Ideally you will be a non-medical independent prescriber but the PCN will actively support you to achieve this, if not already an independent prescriber You will be required to lead by example, setting and maintaining high standards of professionalism within a team environment You should be IT literate and confident in using practice systems About Us We formed our primary care network, to work together to enable us to operate more efficiently and ensure that a greater proportion of our resource is focused on clinical care and improved access Our PCN covers 4 practices across West Essex, Peacock Surgery, Angel Lane Surgery, John Tasker House supporting 54,000 patients.
