The Community Neuro and Stroke Service is looking to recruit a Complex Neurology Clinical Lead B7- 1 WTE permanent, although looking at splitting into 2 part time posts (1x North/West, 1x South). The allocation of hours for each post can potentially be flexible depending on applicants.
We are looking for a suitably experienced individual to work as a clinical lead with individuals with complex neurological conditions. Palliative care experience would be especially useful as there is a significant input into community end of life support with patients.

You will be undertaking comprehensive assessments of the health needs of people diagnosed with deteriorating complex neurological conditions (predominantly MND although will be required to provide intervention with other conditions), and through proactive case management focus on ways to make demonstrable improvements to those individuals' experience of the condition and quality of life. You will take on the role of leading and co-ordinating input to individuals, and taking the lead with co-ordination of the MDT where required.
