Occupational Therapist
Band 6
Integrated Transfer of Care (ITOC) Team
Hours per week: 27.75 – The service operates from 0800 to 1800 across 7 days.
Job Type: Permanent
The ITOC team is a multidisciplinary group including nurses, occupational therapists, physiotherapists, and Assistant Practitioners, working in collaboration with social services and other community services.
The team’s purpose is to support safe, timely hospital discharges for patients who are medically fit but no longer need hospital care, facilitating tailored assessments and care to promote independence. The team also aims to reduce unnecessary admissions in A&E and Frailty departments.
The model supports better outcomes by:
* Reducing hospital stay durations for those no longer needing acute care
* Assessing patients in appropriate environments outside hospitals
* Providing multidisciplinary reablement and rehabilitation plans
* Prioritizing acute hospital care for those who need it most
The service comprises two teams: The Hospital Discharge Team manages onward care needs and discharge arrangements through pathways within the Recovery Hub—Home First (default pathway) and Bedded Pathway. The team works with individuals to determine support needs and arrange necessary support for discharge. Longer-term needs are assessed after stays in the Recovery Hub.
Both teams cover A&E and Frailty patients across Kirklees and there are opportunities to expand scope through support in Unplanned and Intermediate Care services.
