Primary Care Pharmacy Technician, Farnham
We are pleased to offer an exciting opportunity for an onsite Primary Care Pharmacy Technician to join Farnham Primary Care Network on a full-time basis. This role is a 12-month maternity leave cover.

As our Pharmacy Technician, you will play a key role in optimising medicines management, supporting clinical workflows, and helping to ensure safe, effective prescribing across our practices.
Main Duties of the Role
As a Primary Care Pharmacy Technician, you will work within professional standards and defined boundaries, supporting your General Practice team in collaboration with Pharmacists and Pharmacy Technicians across PCN.
You will contribute to resolving medication-related queries, work collaboratively within the multidisciplinary team, enhance patient outcomes, and support initiatives that improve access to healthcare services.
Job Responsibilities
* Conduct medication reconciliation using clinical correspondence and hospital discharge summaries.
* Support effective medicines management, including prescription screening and processing.
* Respond to and communicate with patients regarding medication-related queries.
* Liaise with local community pharmacies to address supply issues and medicine shortages.
* Carry out medication synchronisation to align with regular repeat prescriptions.
* Manage medication monitoring recalls, such as blood pressure checks, blood tests, and annual reviews.
* Undertake prescribing audits aligned with CQC and QOF priorities, as directed by the Prescribing Team (training available).
* Complete routine and annual medication monitoring tasks.
* Work within Shared Care Protocols to support safe prescribing and monitoring.
* Action medicines safety alerts issued by CAS, MHRA, and the ICB.
* Perform Level 1 and Level 2 medication reviews (training provided as needed).
* Assist with pre-SMR (Structured Medication Review) screening processes (training available).
Person Specification
Qualifications
* Registered Pharmacy Technician with the General Pharmaceutical Council (GPhC) (Essential)
* Completion of the CPPE Primary Care Pharmacy Education Pathway (PCPEP), or evidence that you have already started or are working towards its completion. (Desirable)
Experience:
· Experience in primary care or community pharmacy
· Medicines reconciliation and medicines management
· Working within a multidisciplinary team
· Experience using EMIS (desirable)
Skills:
· Knowledge of repeat prescribing processes
· Understanding of medicines optimisation and patient safety
· Familiarity with CQC and QOF standards
· Good communication and organisational skills
· Professional and patient-centred approach
· Attention to detail
· Ability to work independently and as part of a team
