Overview
Senior Band 6 Community Staff Nurse with a Diploma in Integrated Community Nursing or District Nursing qualification. The post holder will be responsible and accountable for the delivery of an accessible, high quality and effective nursing service to the community by working in partnership with patients, care providers and other agencies. You will ensure a person-centred approach to care delivery which is safe and evidence-based to support and maintain quality outcomes.
Responsibilities
* Function as an independent autonomous practitioner to lead and enable the Community nursing team to provide a quality nursing service.
* Work with the Community Nurse Team Lead to lead/manage and support the Community nursing team; act as a clinical expert and senior clinician.
* Be involved in public health activities and be accountable for a caseload.
* Collaborate with multi-disciplinary groups within the Integrated Community team and other partners to deliver holistic care.
* Maintain effective communication, use initiative, and participate in continuing professional development.
Qualifications and Requirements
* Diploma in Integrated Community Nursing or District Nursing qualification.
* Proven ability to work as an independent autonomous practitioner with leadership and mentorship responsibilities.
* Strong communication skills, ability to work from own initiative and in a team; commitment to ongoing professional development.
* Car driver with a full driving licence required due to rural caseload.
