The successful candidate will work within the Frailty Team in the fast-paced Emergency Department and Acute Assessment Unit with opportunities to work on the South-Central Ambulance Falls/Frailty Car project. Frailty Specialists are the role models of exceptional, appropriate and compassionate care of older people living with frailty.
The post is open to all health professions who can demonstrate the skills to assess a patient's physical, cognitive and social ability to remain at home or put plans in place to meet any acute or long term needs.
Main duties of the job
The frailty team work very closely with our community partners and are actively involved in the Hampshire Together programme. We expect that this role will work flexibly across hospital, community and ambulance services in line with the evolving integrated care strategy.

* An understanding of the unique challenges someone living with frailty faces, especially when acutely unwell
* Someone who values working as part of a close virtual multidisciplinary team
* Excellent communication skills
* Previous experience with older adults or those with complex needs
* Advanced assessment and care planning skills using a holistic approach
* A willingness and ability to work hands-on in all aspects of patient assessment and care
* Completed or looking to complete post-graduate education related to Frailty
