About the Role
You’ll be a vital part of a forward‑thinking team dedicated to proactive, preventative care for adults aged 50‑64 who are at risk of frailty. Working across the PCN, you’ll collaborate closely with a dedicated care coordinator to identify, engage, and support individuals early—helping them maintain their health, manage chronic conditions, and prevent deterioration.
What You’ll Do
* Drive early intervention strategies to improve health outcomes
* Provide clinical expertise directly to patients, ensuring personalised, compassionate care
* Work within a vibrant, integrated neighbourhood team alongside health and social care professionals, the NCA, and Oldham Council
* Contribute to a pioneering health programme that puts prevention at the heart of care
