This post is a Fixed Term/ Secondment until 12.05.2027. If you are a Hywel Dda University Health Board employee and are applying for this post as a secondment, agreement for the secondment is needed from your current manager prior to application.

Hywel Dda Health Board present an exciting opportunity, for a motivated, passionate and experienced person. They will join our multi-disciplinary front of house team, TOCALS (Transfer of Care Advice and Liaison Service), as a Clinical Lead Physiotherapist. TOCALS support a large variety of patients, presentations and pathologies, including those experiencing falls and frailty, neurological conditions, cardio respiratory conditions and trauma and orthopaedic problems (including major trauma patients).

TOCALS support prevention of unnecessary admission into an acute hospital to avoid the risks associated with this. The aim is to achieve high quality person centred care which considers the patient holistically, promoting their independence and wellbeing. TOCALS play a pivotal role in promoting excellent links between acute, intermediate, community and third sector services; this is vital to ensure continuity of care, and promote the right care, in the right place, at the first time approach.


Main duties of the job

The successful candidate will require extensive post graduate experience across a broad array of clinical areas, which they can apply to the diverse populations who present to front of house services. They will need to be able to provide highly specialist assessments, with a focus upon discharge planning, from which they can develop and implement co-produced, personalised high quality plans for patients. The post holder will play a key role in following the patients' journeys through hospital, and out into the community.

The post holder will be able to evidence a passion for high quality care and experience of service improvement, particularly where this links with national drivers. Strong leadership, organisational and team management skill are essential, with the need to lead a team in pressurised and fast pace environments. Continuous personal and professional development is essential to ensure TOCALS is responsive to current evidence based practice and population needs. The Clinical Lead Physiotherapist will support leading an in-service training programme for the team, promoting the team to be reflective life-long learners. They will also support staff supervision and annual reviews. Supporting students is key to supporting development of the future Physiotherapy workforce, and the successful applicant will undertake Clinical Supervisor duties.

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Two integrated care centres (Aberaeron and Cardigan, Ceredigion).Community facilities, including:

48 General Practices (GP surgeries), 49 Dental Practices, 98 Community Pharmacies (chemists), 44 General Ophthalmic Practices (including eye health and low vision services), 38 sites providing mental health and learning disability services, care within your own homes.

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.


Job responsibilities

This post will be complimented by the presence of a fellow Clinical Lead Physiotherapist who works Thursday-Friday.

The post holder will be required to participate in the provision of a flexible 7-day service, based on service need and rota basis. They will have the skills required to participate in the Respiratory On-Call Service.

The ability to travel to location in the community is required to support the out-reach services provided by TOCALS. Cross site cover may be required (Prince Philip Hospital).

Qualifications and Knowledge

* Professional registration as recognised by HCPC.
* Significant accredited post graduate specialist physiotherapy courses within relevant clinical area
* Advanced training to Masters level training.
* MSc within relevant specialty
* Evidence of leadership courses/qualifications
* Member of the CSP
* Post graduate qualification up to post grad diploma level in relevant area
* Clinical Education Training course


Experience

* Significant post-graduate experience; including relevant experience in specialist field
* Managing a complex caseload using an advanced clinical reasoning process
* Significant experience working as an autonomous practitioner
* Significant experience in multi agency team working within specialist area
* Therapeutic handling and risk assessments
* Direct involvement with carers and families
* Evidence of supporting service compliance with clinical governance requirements
* Leadership of clinical networks/ clinical practice groups
* Experience in assisting in leading service developments
* Experience in staff development
* Experience in presenting within clinical area
* Performance Management
* Evidence of team management.
* Experience of service development across professional boundaries
* Experience in undertaking risk assessments
* Experience of health promotion and preventative developments within service
* Experience of referring for and analysing the results of tests
* Teaching & educational assessment experience
