Duties
* Coordinate and manage care plans for clients, ensuring that all services are delivered in a timely manner.
* Serve as the primary point of contact for clients and their families, addressing any questions or concerns regarding care services.
* Collaborate with healthcare providers to facilitate communication and ensure comprehensive care.
* Maintain accurate records of client interactions, care plans, and service delivery in accordance with organisational policies.
* Assist in scheduling appointments and follow-up visits for clients as necessary.
* Monitor client progress and adjust care plans as required to meet changing needs.
* Provide support and guidance to families navigating the healthcare system.
Qualifications
* Previous experience in a healthcare or social services role is preferred.
* Strong organisational skills with the ability to manage multiple tasks simultaneously.
* Excellent verbal and written communication skills.
* Proficiency in using office software applications (e.g., Microsoft Office Suite).
* A compassionate approach with a genuine desire to help others.
* Ability to work collaboratively within a team environment while also being self-motivated.
* Knowledge of healthcare regulations and standards is an advantage but not essential.
