Main duties of the job
Your Role
You'll manage a varied caseload across 6 sessions per week, delivering outstanding care to our patients. You will be responsible for:
* Surgery and telephone consultations
* Home visits (where needed)
* Acute and repeat prescriptions
* Preventative health screening and health education
* Managing undiagnosed and chronic conditions
* Creating individualised care plans
* Participating in audits, QOF, and quality improvement
* Supporting trainee GPs and healthcare staff
* Contributing to the success of our Primary Care Network (PCN)
You will work with autonomy but always with the support of a highly collaborative team.

Job responsibilities
The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.
Clinical responsibilities include:
* Surgery consultations, queries, home visits, and signing repeat prescriptions
* Making autonomous decisions regarding presenting problems
* Assessing health care needs of undiagnosed problems
* Screening for disease risk factors and early signs of illness
* Developing care plans in consultation with patients
* Providing counselling and health education
* Referring patients to other care providers as appropriate
* Recording consultation notes to standards
* Participating in audits and data collection
* Prescribing medications appropriately
* Supporting other clinicians and team training
You will also collaborate within the Primary Care Network and adhere to all relevant policies and guidelines. Maintaining an interest in training and education, contributing to team development, and attending relevant training are also expected.
