Advanced Clinical Practitioner – Garioch Cluster
An exciting opportunity has arisen for an Advanced Clinical Practitioner to join our Garioch cluster. We are seeking an exceptional individual who can join us in our continued journey to deliver excellence in care throughput Aberdeenshire.
Responsibilities
* Deliver positive outcomes for patients by providing enhanced clinical assessment and treatment within the community setting.
* Support medical staff and other health care professionals in the community to provide optimum service delivery.
* Work in collaboration with independent practices to provide a high level of care throughout clusters.
* Undertake acute on the day triage and home visits when required.
* Operate across multiple clinical areas including GP surgeries, community hospitals and patients’ own homes.
* Recognise and initiate clinical management of deteriorating patients and facilitate admission to hospital or virtual community ward when necessary.
* Proactively expedite the patient journey and develop the role of the Advanced Clinical Practitioner within Aberdeenshire as part of a multidisciplinary team.
Qualifications and Experience
* Registered professional as an Advanced Nurse Practitioner or Advanced Clinical Practitioner with evidence of advancing practice.
* Sound understanding of conditions that may require admission to hospital or the virtual community ward.
* Experience and clinical knowledge of managing patients in their own home as part of an MDT team.
* Completion of the Advanced Care Academy framework, including a portfolio, within an agreed time frame.
* Valid driving licence and access to a vehicle; willingness to travel within the Garioch cluster.
* Flexible working hours; part‑time hours considered.
