Overview

The role also supports person-centred care planning, by implementing, monitoring, and undertaking monthly evaluations. In addition, the post holder will support residents with their medication requirements and contribute to staff supervision and development where required. They will act as a key point of contact for residents, their relatives and visitors, and for health and social care professionals involved in residents’ wellbeing, ensuring reasonable requests are acted upon and concerns or complaints are escalated.
Responsibilities
* Monitor the wellbeing of each resident and be aware of any changes that impact care and care plans.
* Ensure new needs are met and written records are amended to reflect any variation.
* Involve themselves in the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations.
* Support residents with their medication requirements.
* Undertake staff supervisions and support staff with their development where required.
* Act as a key contact for residents, their relatives and visitors, and for health and social care professionals; ensure reasonable requests are acted upon and concerns or complaints are escalated.
ABOUT YOU
* Live our values of caring, supportive, honest, respectful, and accountable in all you do.
* Have an NVQ 3 or equivalent in care for care homes.
* Have 2 years’ experience in a similar care environment.
* Have the ability to communicate effectively both verbally and in writing.
* Have a positive attitude to supporting and nurturing residents and staff.
* Be responsive and flexible to cover a range of responsibilities.
