The Perth & Kinross HSCP Hospital Discharge Team based within Perth Royal Infirmary is looking for an experienced Registered Nurse to become the Lead Discharge Coordinator. The team are looking for a Registered Nurse with strong professional, leadership skills to support the progression of an integrated discharge service within the PKHSCP and be part of the NHS Tayside wide discharge team integration, in partnership with the existing Social Work Hospital Discharge Team. This post will manage a team of Discharge Coordinators, and Patient Flow Coordinator, and will work closely with the Team Leader for the Hospital Discharge Social Work Team.
It is essential that you can demonstrate excellent interpersonal, communication and organisational skills. You will be innovative and proactive with the ability to be flexible and react positively to the ongoing changing demands and priorities within the services, across the localities within PKHSCP, and collaborate with the wider Tayside discharge teams.
You must be registered on Sub-part 1: RN1 or RNA or Sub-part 2: RN2 of the UK NMC Register
