£35-£38 Per Hour
Role 1
Front Door Team - To provide an outcome focussed assessment service to all adults with physical disabilities or age related problems.
Hybrid working
Role 2
Hospital Team -
• To provide an outcome focussed assessment service to all adults with physical disabilities or age related problems. xxuwjjq
• Local Authority and hospital discharge experience required.
• Knowledge of the Mosaic case management system is desirable
• Hybrid set up, min 2-3 days a week in the office
• Must be a driver with a valid driving license and a vehicle to use for work
